• Your passport and/or original birth certificates will be mailed to our office, and we will inform you to come pick-up.
• Everything must be printed single-sided

1. Go here to fill out a passport application- Passport Application 
   1. Application must be filled in on the website so that the barcode in the upper left-hand corner is present.
   2. DO NOT SIGN until you are in the presence of your passport agent.
   3. DO NOT put your home address under Mailing Address. Put the address below instead.

   4. Passport Products and Fees: Choose Passport Book ($130) (You will not be charged)
   5. Processing Methods: Choose Routine Service (FREE)
   6. Delivery Methods: Choose Standard Delivery (FREE) and then click next
   7. Click that you have read and acknowledged the steps and information contained above
   8. Print Form, pages 5-6, print one-sided and do not sign until in the presence of passport agent

You will need to bring the following items to your passport appointment located in building 170:

1. 1. Renewal Passport  
      1. Passport (expired within the past 5 years) and a photocopy
      2. DTS Orders (DD 1610 approved by Finance with the travel number on the bottom right-hand corner), an approved Memorandum in Lieu of Orders (MILO), or PCS orders
      3. 2 Passport Photos (must follow photo guidelines, recent within 6 months)
      4. Military ID and a photocopy (or License and a photocopy for dependent)
      5. Unsigned DS-82 (print one-sided) 

1. 10. Initial Passport  
       1. Original Birth Certificate or other proof of US citizenship and a photocopy
       2. DTS Orders (DD 1610 approved by Finance), an approved Memorandum in Lieu of Orders (MILO), or PCS orders
       3. 2 Passport Photos (must follow photo guidelines, recent within 6 months)
       4. Military ID and a photocopy (or License and a photocopy for dependent)
       5. Unsigned DS-11 (print one-sided)
